Your Medical Record
At Full Spectrum Psychiatry, we utilize a web-based electronic medical records system called Sessions Health to maximize your care. Sessions is HIPPA compliant and uses bank-level encryption to protect all medical records.
The online patient portal provides:
- 24/7 online access to your account and select medical information at home.
- Secure messaging with Dr. Borue.
- Real-time access to Dr. Borue’s appointment schedule.
- Scheduling/rescheduling of various appointment types.
- Billing and payment (Please note that Dr. Borue requires a valid credit card number to be kept on file).
Electronic Paperwork
All referrals to Full Spectrum Psychiatry must be submitted through our online form. Referrals are typically reviewed within 3-4 business days. After your referral is processed, you will receive an email with a Sessions Health portal invitation as well as instructions for scheduling, filling out intake paperwork, and completing your patient portal setup . Please keep a look out for emails from Sessions Health and set up your spam filter to ensure that these messages get through to your inbox.
Intake paperwork must be completed at least 24h prior to your scheduled appointment to allow Dr. Borue time to prepare for your visit. Follow-up paperwork for patients attending their medication management or therapy appointments must be completed prior to the start of your appointment.
Responses to intake and follow-up questions allow Dr. Borue to focus your visit on the most relevant concerns. They will be kept as part of your record and used to track your progress over time. Any written records will be scanned and stored as a pdf attachment within your medical record and the originals will be shredded.
Please Note: If a patient portal account is not established and all requested documents are not completed 24h PRIOR to your intake appointment or prior to the start of your follow-up appointment, it will be necessary to reschedule for a later date/time to allow time for the completion of this necessary documentation. Our cancellation/ no show policy will be in effect in such circumstances.
About your privacy:
Full Spectrum Psychiatry has a signed BAA with Google to ensure the safety, security, and HIPAA compliance of all electronic Docs, including this form. All forms are accessed through a HTTPS (Hypertext Transfer Protocol Secure) connection. Google and Full Spectrum Psychiatry use safeguards to prevent against unauthorized use or disclosure of your protected health information. Google Docs (Forms) are audited using industry standards such as ISO 27001 certification and SOC 2 and SOC 3 Type II audits, which are the most widely recognized, internationally accepted independent security compliance audits.
